Personalised care means making sure support is tailored to you as an individual – not just your cancer treatment. Living with cancer can affect many areas of life, so personalised care looks at your physical health, emotional wellbeing, family, work, and everyday activities.
A key part of this is the Holistic Needs Assessment (HNA). This is a simple questionnaire or conversation that helps you and your healthcare team identify what matters most to you, any concerns you may have, and the support you might need. It covers areas such as symptoms, emotional health, money worries, work, relationships and lifestyle.
From your HNA, a care plan is created. This is a written document that sets out your priorities, the support available, and the next steps in your care. It may include information about treatment, managing side effects, emotional support, or services in the community.
Together, the HNA and care plan help ensure you feel listened to, supported and involved in decisions about your cancer journey.
