Heart Failure Service

Heart failure patients care including management of symptoms and heart failure medications, education on self care, counseling and support for patients and families and Carers.

For patients who think they have heart failure GPs may refer you to our new fast access heart failure service. Just see your GP and they will be able to refer you. Criteria for referral includes a special blood test called BNP or if you have previously had a heart attack.

Who are the services for?

  • Heart failure nurses
  • Patients with a known diagnosis of heart failure
  • Under a cardiologist here at Portsmouth
  • For the fast access heart failure service via GP
  • patients with symptoms of heart failure who have either had a heart attack in the past or have a raised BNP level in a blood test.
