Discharge planning starts as soon as your baby is admitted to the Neonatal Intensive Care Unit. Staff will prepare you and your family as much as possible, well in advance of your baby's discharge. As your baby's condition improves, he/she will move to different rooms on the unit, please don't be alarmed if you find your baby is not where you left them when you arrive on the unit to visit!
As soon as you have received your Parent Held Record from your Health Visitor for your baby, please bring it in and give it to the nurse looking after your baby. We can then start to document information about your baby's stay on the unit for you and other health professional's future reference.
It may be necessary for your baby to have a follow-up appointment once discharged. This may be undertaken by the Community Neonatal Nurse in your own home, or as an outpatient at the hospital. Any follow-up appointments will be explained to you prior to discharge.
The NICU staff will be able to give you the addresses and telephone numbers of support groups should you need them. Please feel free to ask.
