According to Standard 6 of the National Services Framework for diabetes, all young people with diabetes should experience a smooth transition of care from paediatric diabetes to adult diabetes services via a Young persons clinic. In keeping with this standard, Portsmouth and Gosport Peninsula have a young persons clinic for diabetes.
Typically the youngest patient are aged over 14 years while the oldest approximately 20 years - though there is no fixed age barrier to entering this clinic.
